Connected Care in the Great Southern - Albany Roundtable

Join WA Primary Health Alliance at a collaborative evening event designed to strengthen connections between primary care and the aged care sector across Albany and the Great Southern.

This event will bring together residential and community aged care, general practice teams, allied health professionals and key sector agencies to explore best practice and identify opportunities to improve access to coordinated aged care through:

• Identifying practical opportunities to strengthen collaboration and local referral pathways between residential aged care, community providers and general practice.
• Analysing examples of best practice clinical models to determine how they can be adapted locally.
• Examining General Practice in Aged Care Incentive (GPACI) principles through real-world scenarios that support timely access to clinical support for aged care residents.
• Demonstrating how telehealth workflows can be integrated to minimise disruption for residents and streamline provider communication.
• Identifying local barriers and enablers to improve access to integrated care.
