We identify priorities, link services, and fund care that is locally accessible, by working together with GPs.
We’re committed to collaborating with general practices across the state to develop and implement quality treatment and support plans, services and activities that help them meet the unique health needs of their local communities.
We do this by:
- Providing face to face and virtual visits to general practices, as well as providing a Help Desk service to all general practices across the state
- Offering a range of quality improvement activities, including holding webinars and workshops
- Providing data sharing reports to better understand their patient population
- Sending newsletters with important clinical and practice management updates.
