ARBOR (Active Response Bereavement Outreach) is a proactive postvention support service providing short to medium term bereavement counselling and support for people bereaved by suicide. Office-based and outreach services are provided in the metro area by professional grief and bereavement counsellors and trained peer supporters. Peer supports are individuals who have been bereaved by suicide in the past. ARBOR is active in the community following suicide and aims for early engagement of those bereaved (family, friends, professionals and peers) with support services, resources and assistance. The program aims to: (1) reduce stigma and isolation associated with suicidal bereavement and normalise the grief process; (2) reduce the risk of suicide amongst those bereaved; and (3) raise awareness of the complexities of suicide bereavement with allied health professionals.

The staff of this service work collaboratively with the Aboriginal Outreach Worker from Womens Health and Family Services to address the needs of the whole family and allow direct access to other programs delivered by their organisation. Together and independently, the male and female Aboriginal Outreach Workers in the two organisations host community engagement activities with the broader Aboriginal community in these areas, to build trust and relationships, which assists in determining the types of interventions required.

The Attachment Program is a program targeting, primarily women, mothers to be and other primary caregivers with a history of problematic substance use. Program activities are designed to open communication, assist the mothers knowledge of child development and improve the mother-child relationships. The program is delivered by qualified counsellors and arts therapists who work with the primary care-giver and their children (up to 12 year of age) to build positive and secure relationships. A variety of therapeutic approaches are used including art therapy, play therapy, psycho-education and counselling focused on attachment and parenting. The therapeutic approaches are delivered in family and group programs alongside of individual counselling sessions to address the impact of their substance use on their children.

The Mental Health Nurse will support people who use the Aboriginal Medical Services in the Ngaanyatjarra Lands. Within their scope of practice, they will assess all levels of mental health illness and its impact on the life of the individual, their families, carers and their community. It is to provide treatment that is gender specific, locally and culturally appropriate with developed client self-management plans and client directed recovery plans. They will work alongside GPs and allied health professionals to provide a cohesive multidisciplinary approach to care and provide client /carer advocacy. This program will reduce the likelihood of unnecessary hospital admission and readmission for people with mental illness. It will assist in keeping Aboriginal and Torres Straits Islander people with mental illnesses well on country and feeling connected to the community by addressing unique barriers faced in this remote location including cross boundary care and after-hours services. In the absence of a GP, the Mental Health Nurse Practitioner can prescribe medication, within scope of practice, administer, monitor ensuring compliance. This Activity provides a stepped care approach and supports clients to transition up or down as their needs change.

Overall, the activity will provide care management for people with severe and complex mental illness who can be appropriately managed in the primary care setting by a General Practitioner (GP), Psychiatrist and/or other appropriate medical practitioner. All referrals to the face to face care management service will be by a GP, Psychiatrist or other appropriate medical practitioner, either directly or through the telephone based service provider. Initially, the service will focus primarily on those people who are currently accessing services under the Mental Health Nurse Incentive Program (MHNIP) and meet the severe and complex criteria. In a situation where a person currently accessing services under the MHNIP scheme does not require ongoing care management, the service provider will facilitate a warm referral to an appropriate service or develop a suitable exit strategy. The care managers will be a mental health nurse or other tertiary qualified mental health practitioner and will be assigned to work with specific people. During the period of intervention, this time-limited care management service will allow GPs to safely refer people into structured face to face care management that is closely linked with local health and social services. This service will work closely with the people accessing the service and their GP to prepare, review and implement their mental health care plan and prepare them for their GP consultations. The service will prioritise and proactively apply a solution focused approach to collaborate with, and refer to, local place based supports that best address the needs of peopleas part of a Stepped Care and shared care approach. The provider will also need to work closely with WAPHA and the telephone based service provider to develop and expand this face to face care management service over time in Metro areas with identified need, on a planned and phased basis, in cooperation with local GPs and other health professionals The

This Activity is an Aboriginal youth diversionary project designed and governed by elders from within a cultural block in the south west Kimberley that includes Nyikina, Mangala, Karajarri and Walmajarri language regions. This area extends from Bidyadanga in the West Kimberley to Wangkatjungka in the southern (central) Kimberley. This Activity provides an on country, cultural program to build individual, family and community resilience and well-being. This Activity is carried out by the Contractor and Aboriginal elders who take Aboriginal youth on ¢â?¬Ë?back to country trips to their traditional lands. During these trips, that are grounded in traditional law and culture, Aboriginal youth participate in cultural activities that enhance their understanding of their culture, cultural sites, their roles, importance and connection to country and community. Young people become healthy when: they spend time on the country of their ancestors and family; they are taught to speak in the traditional languages of their family; they are given opportunities to be deeply immersed in the culture, law and stories of their ancestors and family; and they are given opportunities to carry out the above in conjunction with members of their parents and grandparents generation through a process of intergenerational transmission. The Contractor will assist Aboriginal youth in a culturally appropriate manner through collaboration with the wider Fitzroy Valley community and relevant agencies to address issues of self-harm and suicide.

St John of God Outreach Services – Raphael Services, offer early-intervention counselling, therapy and support to enable families to thrive emotionally and physically. Raphael Services multidisciplinary model, overseen by a Consultant Psychiatrist, includes specialist mental health clinicians, delivering personalised, community-based care to families. The mental health nurse is an integral part of the multi-disciplinary team (MDT) within Raphael Services. In collaboration with general practitioners (GPs), Raphael Services provide secondary level specialised mental health services to families experiencing mental health issues during the perinatal period; from conception until the child is four years old. Services are tailored to client needs and can include: (1) Assessment clients are offered an initial assessment. Appointments are made at the closest Raphael Services location with one of the experienced mental health clinicians to assess the presenting mental health issues, discuss individual needs and work together to agree on a treatment/support plan. All clients are discussed at the MDT meeting following assessment, and an overview and plan communicated back to referring GP; (2) Individual therapy one-on-one sessions with a mental health clinician; (3) Group therapy groups provide an opportunity to connect with other parents who are experiencing similar feelings and challenges. Raphael Services facilitates the Circle of Security Parenting Program, aimed at improving the parent-infant relationship; and (4) Psychiatric support clients are offered a psychiatric medical appointment, including advice and prescribing of medication, where indicated. For this initiative, the definition of mental Illness is consistent with that applied to MBS Better Access items. People with dementia are not excluded from treatment if they also have a comorbid mental illness such as anxiety or depression. Delirium may present with symptoms similar to those associated with a mental illness although it will not respond to psychological therapies and requires urgent medical assessment. The Activity will provide post-treatment and aftercare services, including psychosocial and living skills support, to individuals exiting intense Alcohol and Other Drug (AOD) treatment (residential or outpatients) to enable them to maintain treatment gains, reduce relapses and reliance on AOD and increase adherence to relapse prevention plans. For the purpose of this Agreement: (1) Post-Treatment is defined as the stage of treatment where the service provides maintenance support for people who have completed initial, more intensive treatment and could involve provision of counselling for ongoing issues; (2) Aftercare aims to facilitate psychosocial and living skills support for persons exiting AOD treatment and commences once the intense treatment has finished and the client has achieved her/his treatment goals. Aftercare can be provided simultaneously with Post-Treatment and the service intensity will vary depending on client needs, but may include: (a) Booster sessions and/or follow up meetings; (b) Support to attend self-help groups (e.g. Alcoholics Anonymous or Narcotics Anonymous); (c) Information about community engagement opportunities (e.g. interest groups, social networks); (d) Participation in regular sessions with other aftercare peers; and/or (e) Support to access a range of health, welfare and ongoing support services (such as housing, legal, vocational, mental health services, primary health care, etc.).

The South West Aboriginal Suicide Prevention Service is an evidence-based, holistic, culturally appropriate and safe, suicide prevention treatment service for Aboriginal and Torres Strait Islander people at risk of suicide in the South West Region. The Service will be provided by an appropriately skilled workforce via face to face, telephone and virtual modalities and it will improve integration of primary mental health care and suicide prevention services by developing and strengthening appropriate partnerships and referral pathways. The Service builds the capacity and capability of the local health and primary care services regarding suicidal behaviour and the treatment of Aboriginal and Torres Strait Islander people in a community-based setting. The service will also build the capacity and capability of the local Aboriginal and Torres Strait Islander community to strengthen the individuals, family and community to better identify and support those who are at risk of, or who have attempted suicide or self-harm. The Activity connects to, and compliments, existing activities which support the social and emotional wellbeing of the Aboriginal and Torres Strait Island community in the South West.
