About this opportunity
The General Practice in Aged Care Incentive (GPACI) – Practice Based Care Coordination is a grant opportunity to provide uplift to general practices based in the City of Busselton local government area (LGA) to increase capacity and capability to support and/or strengthen relationships to improve care of residential aged care home (RACH) residents.
Objectives:
- Identify existing gaps in the delivery of primary care services to RACH residents.
- Collaborate with local RACHs, GPs and other allied health providers, general practices and/or Aboriginal Community Controlled Health Services (ACCHS) and providers to design, implement and manage locally informed solutions addressing gaps.
- The grant supports activities as directed by the Australian Government Department of Health, Disability and Ageing General Practice in Aged Care Incentive program guidelines.
