WA Primary Health Alliance (WAPHA) is seeking to commission a suitably qualified provider to provide a formative, mixed methods (both qualitative and quantitative data) process evaluation of the Aftercare Services Program.

The evaluation aims to determine the extent to which the Aftercare service is being implemented as planned, capture lessons learned and provide recommendations for continuous improvement and future scaling.
