GPACI aged care coordination

Upcoming grant opportunity

General Practice in Aged Care Incentive (GPACI) aged care coordination

About this opportunity

WA Primary Health Alliance (WAPHA) is preparing to launch a new grant opportunity for general practices in Busselton, Bunbury, Geraldton and Albany to support improved coordination of primary care for residents of residential aged care homes (RACHs).

The grant will support practices to build their aged care coordination capacity by establishing a new role or expanding an existing role. Funding is intended to support practical day-to-day activities that support consistent delivery of care to RACH residents, and responsibilities for this role may include:

  • using practice systems, templates, recalls and reminders to support completion of GPACI and care planning requirements
  • coordinating appointments, GP and/or nurse practitioner visits, case conferences and telehealth sessions
  • coordinate information exchange following hospital presentations and discharge events to support continuity of care and reduce delays in follow-up.
  • identifying and implementing practical improvements to aged care workflows within the practice.

Practices will also be required to identify and implement at least two sustainability initiatives to support the continuation of aged care coordination activities beyond funding.

Applications will be assessed against predefined criteria, including alignment with the grant objectives, anticipated impact for aged care residents, implementation approach and sustainability. There is no minimum grant amount, with practices able to apply for up to $100,000, commensurate with the scope and scale of their proposal.
