Mental Health Multidisciplinary Team into General Practice – Perth South PHN
Background
WA Primary Health Alliance (WAPHA) is seeking to commission an experienced provider to deliver a Mental Health Multidisciplinary Team model embedded within general practices across the Perth South PHN. The service will initially focus on areas with higher unmet need, including Gosnells, Armadale, Kwinana and Mandurah, with potential to scale across the broader region over time.
The successful provider will be required to establish and deliver a multidisciplinary team that works with general practices through shared care arrangements, integrating with existing clinical workflows. The model is designed to strengthen primary care capability and provide coordinated, GP-led support for people with severe and complex mental health needs, with a focus on continuity of care, service integration and improved outcomes.
While general practice is the central platform for care coordination, access to the service will not be limited to GP referral. In conjunction with the GP component, the provider will also work collaboratively with Aboriginal Community Controlled Health Organisations (ACCHOs), hospitals, and other relevant health and community services.
This includes establishing clear and referral pathways, supporting shared care and information exchange, and facilitating coordinated, integrated care. The model is intended to ensure multiple appropriate entry points (including self-referral where suitable), seamless transitions between services, and alignment with existing service offerings to avoid duplication
