Residential Aged Care Homes (RACH) Telehealth and After-hours Activity
About this opportunity
WA Primary Health Alliance (WAPHA) is seeking Expressions of Interest (EOI) from suitable organisations to engage in the Residential Aged Care Home (RACH) Telehealth and After-hours Activities program.
The EOI will inform a Pilot approach, to support and build capacity of residential aged care homes across the Perth North, Perth South and Country WA PHNs.
Selected RACHs will receive a grant of $20,000 to enable change management and implementation of telehealth. Participating RACHs are required to commit to a series of change management workshops, nominate a change champion and obtain leadership support to participate and sustain impact. The pilot will run between November 2026 and February 2027.
