Perth South PHN

We support primary health providers – GPs, allied health professionals and other community services – to provide coordinated, efficient and effective medical services to patients, particularly those at risk of poor health outcomes.

We aim to achieve these objectives by working directly with general practitioners, other primary health care providers, area metropolitan health services and hospitals to ensure improved outcomes for patients so they receive the right care in the right place at the right time.

We cover south and south eastern areas of the Perth metropolitan area, from the Swan River down to Mandurah, Murray and Waroona and east to Armadale/Pinjarra.PerthSouthBreakout
