Multimorbidity and increased risk of cognitive decline

Chronic diseases are the leading cause of death in Australia and New Zealand and multimorbidity is the “norm”, not the exception among patients with chronic disease. Increased risk of adverse events due to polypharmacy, depression and functional impairment are just some of the burdens of multimorbidity on the patient if gaps in primary care practice are not addressed. This webinar series will cover multimorbidity care models, navigating guidelines, patient priorities and goals, individualised management, interviewing hacks and polypharmacy using four common patient profiles.
