Greater Choices – Palliative Care Champions Project – Perth North
About this Opportunity
WA Primary Health Alliance is offering grants to general practices across Western Australia through the Greater Choices for At Home Palliative Care initiative. The Palliative Care Champions project supports general practices to strengthen advance care planning and palliative care capability through quality improvement activities and use of evidence-based tools.
Successful practices will receive funding to nominate a member of their team to act as the Palliative Care Champion. WAPHA will provide dedicated training and support to build consistent systems and processes within their practice. Practices will gain access to quality improvement resources, specialist clinician expertise, and tools to enhance Advance Care Planning and identify unmet palliative care need to improve patient care.
We are seeking general practices that:
- Are committed to building Advance Care Planning and palliative care capability within their practice.
- Have a staff member who is willing and has capacity to be a palliative care champion throughout the life of the activity.
- Are using Primary Sense in their practice and are willing to implement Primary Sense tools.
- Commitment to promoting equitable access to care, especially for underserved populations.
