About this Opportunity:
WA Primary Health Alliance (WAPHA) is seeking to establish a network of service providers to deliver the Care Finder Service across Country WA PHN to meet the objectives as determined by the Australian Government Department of Health, Disability and Ageing. The role of the care finder is to provide specialist and intensive assistance to senior Australians who need intensive support who could otherwise fall through the cracks to access aged care and connect with other relevant supports in the community.
Care finders are to target individuals who have one or more reasons for requiring intensive support to interact with My Aged Care (either through the website, contact centre or face-to-face in Services Australia service centres), and/or access aged care services and/or access other relevant supports in the community.
