Request for Quote – Multicultural Health Navigation Service – Perth North PHN
About this opportunity:
WA Primary Health Alliance (WAPHA) is seeking to contract suitably qualified providers to deliver a Health Navigation Service to improve access to primary health care services for people from multicultural backgrounds.
The services will:
- Address challenges and barriers faced by people from multicultural backgrounds when accessing primary health care (for example, but not limited to, language barriers, low health literacy and navigating unfamiliar systems).
- Promote integration and coordination between services for people from multicultural backgrounds.
- Improve patient and community awareness and health literacy for people from multicultural backgrounds.
- Support general practices to improve services or arrangements for people from multicultural background.
The services are to be delivered for 1 year in line with current available fund.
