Background

WA Primary Health Alliance (WAPHA) is seeking expressions of interest from suitably qualified service providers to deliver Diabetes Digital Health services across Perth South, Perth North and Country WA.

Services are to be delivered to Western Australian residents aged 18 or over with diabetes who need help managing their healthcare needs and improving self-management skills.
