Midwest
The program delivers culturally safe, evidenced based mental health treatment and support services including direct counselling, advice, advocacy, and educational programmes regarding mental health and wellbeing to Aboriginal people. The service is delivered by an appropriately skilled workforce and adopts a holistic approach that includes physical, mental, spiritual, cultural, emotional and social wellbeing. The activity operates in partnership with Carnarvon Medical Centre (CMC) and Carnarvon Medical Service Aboriginal Corporation (CMSAC).

This activity is for the employment of a full-time Chronic Conditions Coordinator to focus on chronic disease in the country WA pilbara region. In partnership with WA Country Health Service (WACHS) Pilbara, the Chronic Conditions coordinator supports the WACHS Chronic Conditions Prevention and Management Strategy 2015-2000 as well as WA Primary Health Alliance’s (WAPHA) Integrated Chronic Disease Care (ICDC) Activity and the Integrated Team Care program in the Pilbara.
