(Navigation and Coordination Service)

CLOSED: Peel Youth Mental Health Navigation and Coordination Service

Background:

The Peel Youth Mental Health Navigation and Coordination Service is a co-commissioned service, equally funded by WA Primary Health Alliance (WAPHA) and the WA Mental Health Commission (MHC).

This project targets young people 12-25 who have a mental health condition, whether diagnosed or not, experienced alongside one or more biopsychosocial complexities impacting their care, and who are not currently engaged with a service.

The ‘Peel Youth Mental Health Navigation and Coordination Service’ will be delivered to improve consumer experience, trust and engagement, improve system efficiencies and address the ‘missing middle’ in the service provision continuum.
