Team Based Advance Care Planning and Palliative Care for General Practice

The Advance Project is a Australian Government funded project which aims to support GPs, nurses and practice managers provide better primary health. It supports practices to work as a team to initiate advance care planning (ACP) and palliative care in everyday general practice.

This presentation will familiarise participants with the project and will demonstrate the importance of early ACP and Palliative Care and how you can easily embed into your everyday practice each step of the way.

Presented by Kelly Arthurs RN, Palliative Care Clinical Nurse Consultant, Hammond Care Centre for Learning and Research in Palliative Care.

Cost: Free

CPD: 2 CPD Activity point under the RACGP CPD Program.
