This program works with Aboriginal people, to provide individual case management services to improve access to counselling and to medical and non-medical professional services for people who have mental health issues and problematic alcohol and substance use issues. This program supports person-centred and client focused therapeutic approaches to delivering evidenced based care to Aboriginal people across all ages in a culturally safe environment. It delivers specific mental health programs including suicide prevention, community engagement/education and outreach counselling. The program provides physical, mental, emotional, social and spiritual wellbeing, individual and group counselling, education, psychological therapies and group support during and after treatment. It includes outreach services with the aim to support mental health counselling services and reduce alcohol and drug related harm to the individual and the broader community and assist them to recover on country in a safe and culturally appropriate environment.

This program works with people, including Aboriginal people, to provide individual case management services to improve the access to individual counselling and other professional services for people who have problematic alcohol and substance use issues and mental health issues. This outpatient outreach service provides case management focusing on problems that underly drug and alcohol addiction and relapse prevention. This service increases access rates, maximising the number of people receiving evidenced based drug and alcohol treatment interventions and management. The program provides physical, mental, emotional, social and spiritual wellbeing via medical treatment, individual and group counselling, education, psychological therapies and group support during and after treatment. Outreach services aim to reduce alcohol and drug related harm to the individual and the broader community and assist them to recover on country in a safe and culturally appropriate environment.

The activity delivers a comprehensive range of treatment options to dual diagnoses mental health and substance users and their families in Leonora, Laverton and surrounding communities. It provides an immediate response and has the flexibility to allow the service to be responsive at the time when the clients need the service the most (which is afterhours) and also support the local hospital and community needs. This program delivers evidence-based alcohol and other drug treatment strategies to reduce related harm and improve access to AOD services for individuals and families. It will provides essential coordination with other mental health services to ensure an integrated approach is achieved. This program is supported by therapeutic interventions provided by a registered Psychologist for people referred for mental health counselling. This service has an open-door approach to facilitate referral pathways into cost effective moderately intensive treatment and intervention programs which reduces demand on GPs and hospital emergency department. Therapy sessions can be either one-on-one, group sessions, outreach visits or via telehealth. This program also provides the opportunity to strengthen and support community resilience and capacity by addressing antisocial drug related behaviours.

This activity is for young people aged 12-25 who have experienced a first episode of psychosis to support them in their recovery. This service centres on evidence based recovery oriented practices which recognise that young peoples’ self determination is a vital part of successful treatment and recovery. The service is provided to young people referred from Health Service Providers (HSPs) and is complementary to the care they receive from them.

This Activity supports individuals who have been discharged from the care of the Emergency Department or Mental Health Ward to connect with primary care and other necessary community supports including mental health and alcohol and other drug services in the vulnerable weeks following discharge. Peer support workers work with people discharged from St John of God Midland Public Hospital to improve coordination of care to ensure people receive the right care in the right place at the right time. The activity improves health equity and access by identifying and reducing barriers to people with serious mental illness through short-term person centred peer support.

This activity is delivered by an appropriately skilled workforce, provides time limited intensive case management, provides assertive outreach support to disengaged youth and uses strategies that engage the young person and their family to delay the progression to or prevent the need for ongoing engagement with adult mental health services.

The program provides a service which is holistic, person centred and recovery focussed for young people aged 12 – 25 years with, or at risk of, severe mental illness in the catchment area or community of Kalgoorlie-Boulder. A mental health worker is allocated to undertake a psychological assessment on presentation to the Emergency Department (ED) to identify issues and implement a case management plan. The mental health case worker is responsible for liaison, collaboration and coordination with the client’s GP, and other relevant health and human services to facilitate a joined-up response to meet client needs. In partnership, with the client and their family/carer ongoing review and management occurs by the mental health worker. This program is targeting increase of access to mental health services, early intervention and a stepped care approach to support the client to transition up or down as their needs change. The service uses early intervention strategies which engage the young person and their family to delay the progression to or prevent the need for ongoing engagement with adult mental health services. It is envisioned that this program will reduce readmissions to ED.

The Integrated Aboriginal Mental Health (MH) and Alcohol and Other Drug (AOD) program provides a coordinated system of evidence-based, culturally appropriate and safe mental health and AOD care to address the needs of Aboriginal people in the Southwest of Western Australia. The service will complement and link to existing activities such as general practitioners, specialist drug and alcohol and MH services, suicide prevention, social and emotional wellbeing services, and mainstream services.

Within their scope of practice, they will assess all levels of mental illness and its impact on the life of the individual, their families, carers and community. Provides treatment that is gender specific, locally and culturally appropriate with developed client self-management plans and client directed recovery plans. They work alongside GPs and allied health professionals to provide a cohesive multidisciplinary approach to care and provide client /carer advocacy. This program reduces the likelihood of unnecessary hospital admission and readmission for people with mental illness. It assists in keeping ATSI people with mental illnesses well on country and feeling connected to the community by addressing unique barriers faced in this remote location including cross boundary care and after-hours services. In the absence of a GP, the Mental Health Nurse Practitioner can prescribe medication, within scope of practice, administer, monitor ensuring compliance. This program provides a stepped care approach and supports clients to transition up or down as their needs change.

The Trauma Transformation Initiative (TTI) supports survivors of child sexual abuse to increase access to meaningful support by enhancing their ability to recognise the impact of child sexual abuse and improve their mental health, well-being and relationships. The TTI takes a trauma informed, psycho-educational approach that introduces body based and expressive therapies known to enhance healing and recovery for survivors of child sexual abuse, address complex trauma and improve mental health and well-being.

The Trauma Transformation Initiative (TTI) supports survivors of child sexual abuse to increase access to meaningful support by enhancing their ability to recognise the impact of child sexual abuse and improve their mental health, well-being and relationships. The TTI takes a trauma informed, psycho-educational approach that introduces body based and expressive therapies known to enhance healing and recovery for survivors of child sexual abuse, address complex trauma and improve mental health and well-being.

WA Primary Health Alliance co-funds mental health nurses to provide clinical care co-ordination, structured psychological treatment and low intensity interventions for the UWA population (24,000) with severe and complex and/or high risk mental illness to keep them well in the community and reduce potentially preventable hospitalisations.
