Provision of an after hours GP on-call service to residents of aged care facilities based in Geraldton (Juniper Hillcrest, Opal, and Nazareth House). The on-call GP will provide telephone advice and/or attend in person to deliver primary health care services with the aim of reducing potentially preventable hospitalisations.

The Albany After Hours GP Collaborative service comprises a clinic located within the Albany Health Campus, and a service for residents of residential aged care facilities, or those receiving palliative care in the community.

The Health Navigator service promotes self-management skills to people living with chronic conditions through a virtual service, as required, utilising a modified Flinders My Health Plan.
The service operates in the Great Southern, Wheatbelt and South West. Client eligibility is for heart disease, heart failure, COPD, diabetes or those at risk of chronic conditions.
