HealthPathways WA helps clinicians easily navigate their patients through the complex primary, community and acute healthcare system. It is a free, password-protected website containing localised clinical pathways, which provide clear and concise guidance for assessing, managing, and referring patients across Western Australia.

Home Visiting Service
- Silver Chain (Hedland, Karratha, Roebourne and Newman) Referrals from My Aged Care following assessment. 50+ Aboriginal and 65+ Non-Aboriginal
- COVID-19 Positive Home Visiting and Telehealth Service – medical deputising and nursing services are available to deliver clinical assessment and treatment for people living in residential aged care facilities or private psychiatric hostels across WA, who are isolating due to COVID-19. The service is not intended for routine monitoring and care, but rather to provide one-off or exceptional circumstance consults that will help avoid unnecessary escalation to hospital. Referral is via the resident’s GP, however facility staff can refer if the GP is unable to be contacted.
