Local Patient flow: Assessment, Management and Escalation Guidelines/Pathways
Home Visiting Services

COVID-19 Positive Home Visiting and Telehealth Servicemedical deputising and nursing services are available to deliver clinical assessment and treatment for people living in residential aged care facilities or private psychiatric hostels across WA, who are isolating due to COVID-19. The service is not intended for routine monitoring and care, but rather to provide one-off or exceptional circumstance consults that will help avoid unnecessary escalation to hospital. Referral is via the resident’s GP, however facility staff can refer if the GP is unable to be contacted.
