GPs perform both clinical and legal roles for their patients, including completing Advance Care Directives with their patients, identifying substitute decision makers, and making decisions about emergency or hospital treatment. End of Life Directions for Aged Care (ELDAC) have toolkits to assist in dealing with palliative care or advance care planning in aged care.
