Population health

About 35% of Australians, or 7 million people, have a chronic condition such as heart disease, diabetes, cancer, respiratory disease and mental illness.

The current system is not optimally set up to effectively manage these long -term conditions.

Our work in this area focuses on improving care coordination for people with complex chronic disease and strengthening the capability of general practice through encouraging a multidisciplinary team care approach.

We offer services such as the Integrated Chronic Disease Care program in regional areas and nurse practitioner support in other areas to help people with a chronic condition to navigate the treatment and support system.

We support general practice and other health professionals in keeping up to date with changes to immunisation and screening programs, through our Practice Assist service and our primary health liaison team.
