WAPHA exists to facilitate a better health system, with improved patient outcomes at better ‘value’ to the community.

The Problem

The primary health care system is fragmented and lacks strong, integrated GP led care at its core. Significant access barriers exist for people trying to navigate the system, particularly those at risk of poor health outcomes. These barriers contribute to more than 62,000 Western Australians presenting at hospital emergency departments each year, whose care would be best managed through a co-ordinated and responsive primary health care system.

Plan and commission for quality cost effective services that are sustainable and lead to better health outcomes for people with complex chronic conditions. Engage clinicians and community in commissioning services that meet priority needs and reduce potentially preventable hospitalisations.

What’s Changing?

WAPHA is:

  • Moving from an activity driven health system (focus on episodic and supply driven) to place and consumer centric approaches to new models of care (focus on outcomes and payment by results).
  • Moving to outcomes based commissioning that:
    • Identifies and prioritises the health and service needs of the PHN region
    • Takes a planned approach to prioritised activities
    • Includes competitive dialogue Identifies opportunities for co-commissioning
    • Works in collaboration with stakeholders, clinicians and community representatives to co-design and co-produce and deliver the best possible health care outcomes for Western Australians
    • Recognises the value of the social contract in service delivery
    • Ensures WAPHA’s procurement framework is consistent with, and complements, state models
    • Includes monitoring and evaluation methods which will be developed in consultation with service providers, consumers and general practice.
